specific model number(s) and price(s). Be sure to include your name and address information
for shipping as well as your phone number(s), email address, and fax (if available),
as well as method of payment.


All shipments to a California address are subject to an 8.75% sales tax in addition to the published prices.


If you are paying by credit card (Visa, Mastercard, Discover, or American Express),
please include the completed Credit Card Authorization Form with your order.
